Sewing Class Waiver and Release of Liability

I, [The Parent / Guardian named on this registration], hereby acknowledge that
[Child named on this registration] voluntarily participating in a sewing class offered
by H.O.P.E. -N- HANDS , which involves the use of sewing machines, needles, scissors, and other tools
and equipment.
I understand that sewing involves inherent risks, including but not limited to the risk of injury, accident, or
property damage. I acknowledge that I am participating in this class at my own risk and that I assume full
responsibility for any injuries or damages that may occur during the course of the class.
In consideration of being allowed to participate in this sewing class, I hereby release and discharge
H.O.P.E. -N- HANDS, its officers, agents, and employees from any and all claims, demands, damages, or
causes of action arising out of or in any way connected with my participation in this sewing class.
I understand that this waiver and release of liability applies to any and all claims, whether arising from
negligence, breach of contract, or any other cause of action.
I certify that my child is physically able to participate in this sewing class and have no medical conditions
that would prevent them from participating.
I acknowledge that I have read and fully understand this waiver and release of liability, and I voluntarily
sign it as my own free act and deed.
